Q. Can a patient be on a RightCare© Management Plan and Special Patient Note System at the same time?

A. No. Although a patient can be changed between the systems as their clinical need dictates.


Q. Can a patient be put onto the RightCare© Plan indefinitely?

A. No. Although the patient can be re-registered by sending a new plan every 6 months for RightCare©.


Q. Can I handwrite and fax a RightCare© Plan to you?

A. No. Plans will only be accepted if typed and emailed to us on rightcare.referrals@nhs.net


Q. Can anyone refer a patient onto RightCare©?

A. No. Only the person who has Clinical Responsibility for the patient is able to refer ie. GP, Community Matron, District Nurse, or Specialist Nurse.


Q. Should I automatically put a patient who is on the Gold Standard Framework on the RightCare© system?

A. No. Only when they either reach the Amber stage or they need more active management.
